Can't submit recommendation

I'd like to recommend a researcher. However, I've twice filled out the form at the bottom of his profile page and clicked the Add Recommendation button, and twice the researcher hasn't received anything to approve.
Is there some requirement I might be missing before I can submit a recommendation?

No we don't have any special requirements

hm... can u check the recommendation process itself? i did wonder about some missing recommendations too.
